creating hyperlink form by arcengine into c#

Question

I want to hyperlink three files to one point from a point layer,when I added this layer into Axmapcontroller I need to create a form to show a list of three files in order to choose by user, I could not link this form into c# and link to point . I used arcengine 9.3 and visual studio 2008
