+1 and removed my downvote. I've confirmed "Add as editable layer" works for me here, however editing from Desktop still does not. I can live with that for what we're doing right now. This is a band-aid and not a sustainable solution though; adding all editors as organization administrators is a mistake waiting to happen.

Loading Excel files directly into ArcGIS works wonderfully well -- except when it doesn't! Things to watch for: dates mangled, numbers truncated. First troubleshooting step is always to export to CSV with all columns defined as text and load that instead to see of problem perseveres. That caveat aside. Display as X/Y works very well.

the hybrid idea is very interesting! I'm curious about why a physical file at level-16. Does that offer something gdaladdo foo.vrt 16 32 64 doesn't? ...it also occurs to me: both tiled and vrt overviews might be wanted in instances where individual tiles are access as well as the virtual mosaic.

Gdal does support .rrd, see gdaladdo flag --config USE_RRD=YES|NO. Though maybe it uses the same extension for a different internal structure? The nub of the answer is dead on though, open the tiff; the other files are side-car metadata.

Thanks for Integrate. I didn't like how FeatureToLine moves vertices unpredictably and breaks things into smaller pieces. I've been given the "time to move on!" signal from supervisor so won't be following up on the arcobjects portion right now. This part of the bigger project has been put to sleep. :-/

What kind of hardware and web server setup you need for 5k/s is something out of my league, and very challenging. For instance the absolutely smallest likely web response, even if a static text file, is one Ethernet packet (~1,500 bytes) then 5k/s works out to 60 megabits per second in bandwidth alone (ref). What you're talking about is expensive -- in terms of money to run and time/intellectual effort to build.

dem tiles: A DEM is just a specialised kind of raster. Starting points would be gdal2tiles.py and gdal_retile.py for building the dataset. Any gdal ogr -using map server should be able to read the result.
