I note that after adding the unique ID field directly in SQL Server, I don't see the New Query Layer dialog in ArcMap. Instead, the table is simply added, which is what I was expecting.

@Vince yeah I think you're right. Preparing to use query layers says "The query layer definition must include a unique, not-null column or combination of columns that can be used as the ObjectID"

You've created the map object with the extent in EPSG 102726, but you've also specified the basemap, which is expecting Web Mercator projection. I believe this may be the cause of the projection issues. What happens if you omit the Esri "streets" basemap?

I agree with this. Exporting from Model Builder to Python is already difficult, and buggy. Expecting this to work without knowing Python, while also translating to another language (which you also don't know well) sounds impossible. I'd recommend just starting from scratch in QGIS.
