Maptiler will work directly with Openlayers/Leaflet too. Also if you do the tilemill way, just stretch your image to spherical mercator pixel extent and projection won't be an issue.

if there was some attribute about the start point that made you know it was the start than I see a way. Otherwise like others this doesn't make any sense since direction is arbitrary if there's no rule as to what the right way is.

Why not store the layer as a view in a database, and use your csv creation script to instead import the data to table as part of the view, the other part being the spatial component?

So you want to create create a line network of all points to all other points? GRASS v.net stuff comes to mind for creating the topology which can then be exported to other network analysis tools (though many of them have special formats)

Depends on how complicated your database is. Worst case scenario you have to dump and load your databases. Note postgis 1.5 and 2.0 can exist on the same system.

This method also requires adding the repo key or you'll get a warning and possible a fail that the repo can't be authenticated. sudo apt-key adv --keyserver keyserver.ubuntu.com --recv-keys 314DF160 OR use the ppa method

Yes if you're using ubuntugis-unstable the current version of Postgis is 2.0. However it won't force you to upgrade to that unless you specifically ask it to because such an upgrade requires upgrades to several dependencies.

Last I heard, the File GDB does not have write yet do to a bug in the Arc API (I believe I saw this on the github page for the driver).
