If not for using x/y/rotation columns in your layer's attribute table, how else would the label's location, or proximity to the proposed anchor point, be stored? BTW, anchor points for labels are not yet implemented, but have been discussed.

Well, a polygon will be clipped to the visible map extent prior to being labelled (unless using 'whole' polygon for centroid labeling). If the polygon has a complex perimeter, e.g. shoreline, then the extent clipping may create an invalid, but render-able polygon, i.e. not labeled, though. In upcoming 2.4, there are routines to try and fix such polygons on-the-fly, allowing for more labels to be shown.

There is an issue with the latest GDAL.framework python setup. See: gis.stackexchange.com/a/99841/6725, and apply the fix to the gdal-py2.7.pth file. See if that helps; regardless it is a required fix for now.

Did you check GRASS plugin in Plugin Manager, i.e. enable it? The toolbox is for the Processing plugin, and the GRASS algorithms there are unrelated to the GRASS plugin. However, both use the same GRASS 6.4.3 install, which is bundled within the Kyngchaos.com QGIS.app for 2.2 release.

This will not break QGIS. GDAL is merely a dependency. QGIS may not launch if the shared plugin fails to load, and GDAL throws an app-crashing error. To check that the plugin is loading properly, simply issue ogrinfo --formats and look for any dylib issues. This is part of Step #5, noted above.
