Please see the revised update. I have tried this in spatialite-gui, as well as in QGIS DB Manager. Both methods have produced the same results; added and updated tables that can be viewed in QGIS DB Manager, Qspatialite and spatialite-gui, but not when the table is loaded into QGIS and the attribute table is opened. It is my belief that QGIS has an issues with columns that are added via a different means, other than in the QGIS attribute table. I have had this issue in other cases.

Thank you Brad, I wasn't sure if I needed to make a Virtual table, or if I could import (append) directly into my spatialite table from a shapefile, and limit the columns on the import.

Brad I have added everything you asked for. The only thing I haven't told is that I added the table into QGIS straight from Qspatialite plugin. I have also added the table directly from the Spatialite import in QGIS. What you see is everything I see and have done. What it is showing is that the same table has different column results two packages (spatialite-GUI and QGIS). Is this a known problem with using spatialite?

Yes, but the geometry is in a column called geom. City, state, and country are all text fields.

My spatialite table has three columns (city, state, country). The shapefiles have the previous columns, but also many other columns. I was thinking that my Append would have a SELECT clause in it (ie: SELECT city, state, country FROM shapefile...)
