Thank you! This is the only solution that actually works so far! I would still love feedback on how to get it working in PostGIS, but this does the trick for now!

comment Is it possible to generate a boustrophedon pattern which fill a vector Region with GRASS?
Interesting question. How would you determine the starting point? "Top Left" may not always relate to a nice corner point as in your example.

Thanks. Due to a prior processing step, most of the smaller polygons are in fact already unioned!

Thanks. I just tried ANALYZE, VACUUM and increasing shared_buffers and effective_cache_size, and had the same issue. I'll keep tweaking as time allows.

I'm now having an issue similar to this: stackoverflow.com/questions/17439703/… where the query will start, but after some time will lose the connection. The comment there suggests it's also a memory issue. I'm stuck!

@simplexio as mentioned above, there are no multilines. All features in both tables are simple LINESTRINGS.

John; yes, I want the union for every combination of column1,2 and 3. I'm not sure how to quantify 'large' - but it is a number of very complex (many vertices) polygons with messy overlaps and islands, etc. I'll have to do some research into 'explain' before I can answer your last question!
